  1. *An overall early and late onset disease (EOD and LOD) incidence rate for Dominican Republic was not calculated because Site 1 (maternity) used a hospital-based denominator and Site 2 (pediatric) used a community-based denominator. **The three EOD cases recorded at Site 2 (pediatric) were not included in incidence calculations as cases were from different hospital birth cohorts without a clear denominator. *** Incidence for Bangladesh is not represented as no cases were identified among 4227 births.
